OPTIONAL BENEFITS Optional benefit plans are offered to employees by various carriers. The costs for these plans are paid for entirely by the employee. A summary of benefits for each plan is available below, however; for more information about these plan(s), please refer to the plan benefit booklet. You must visit with a benefit representative to sign up and if applicable when terminating coverage. AFLAC Aflac offers plans such as Hospital Protection and Short-Term Disability. Aflac pays cash benefits directly to you unless you choose otherwise. This means that you will have added financial resources to help with expenses incurred due to medical treatment, ongoing living expenses or any purpose you choose. Please ask the HR Administrator for brochures if you would like additional information on these products. HOSPITAL PROTECTION This plan complements your existing benefit offering by helping with hospital-related expenses not covered by major medical and works well with other supplemental coverage. Cash benefits are paid directly to the employee to use as they see fit. There are no deductibles, lifetime maximums or price increases due to age.
